Oishiii Sweets Cafe will require all DINE IN customers to complete the following form for the sole purpose of contract tracing. Our team will use this information to reach out to any customers in the event of any positive COVID exposures at our location. ONLY 1 FORM IS REQUIRED FOR EACH GROUP
